PREP Reconsideration Request (Capstone Evaluation Result)

This form should be used to request reconsideration of a Capstone Evaluation result that is identified a decision that is subject to reconsideration and appeal under the PREP Reconsideration and Appeal Policy. Students must complete the reconsideration process before being eligible to proceed to appeal. Reconsideration requests must be complete at the time they are submitted. CPLED will not consider information or documentation submitted after the student submits a reconsideration request unless such information or documentation is specifically requested by CPLED.

This form must include all relevant information supporting your request. It is not sufficient to simply state your disagreement with your Capstone Evaluation result or that you want to receive a different Capstone Evaluation result. Provide a detailed statement setting out the specific facts and evidence which speak to how your Capstone Evaluation result was subject to objective, demonstrable, and significant error, a procedural fairness issue, or a failure to accommodate.

Detailed submissions regarding personal circumstances or history are not considered helpful unless such an explanation is directly relevant to the grounds for reconsideration.

Grounds for Reconsideration – Failure to Accommodate
Set out the basis for your reconsideration request. It is not sufficient to simply state your disagreement with the decision made or that you want to receive a different decision. Provide a detailed statement setting out the specific facts and evidence which speak to how the decision was made in error or without procedural fairness such that a different outcome should have occurred. CPLED does not accept reconsideration requests regarding the merit of first attempt Capstone Evaluation results. Students requesting reconsideration of a first attempt Capstone Evaluation result on the basis of an alleged failure to accommodate must limit their reconsideration request solely to the accommodation issue.
