PREP Reconsideration Request (Registration or Disciplinary Decision)

This form should be used to request reconsideration of a registration or disciplinary decision by CPLED that is identified a decision that is subject to reconsideration and appeal under the PREP Reconsideration and Appeal Policy. Students must complete the reconsideration process before being eligible to proceed to appeal. Reconsideration requests must be complete at the time they are submitted. CPLED will not consider information or documentation submitted after the student submits a reconsideration request unless such information or documentation is specifically requested by CPLED.

Grounds for Reconsideration*
Set out the basis for your reconsideration request. It is not sufficient to simply state your disagreement with the decision made or that you want to receive a different decision. Provide a detailed statement setting out the specific facts and evidence which speak to how the decision was made in error or without procedural fairness such that a different outcome should have occurred. Detailed submissions regarding personal circumstances or history are not considered helpful unless such an explanation is directly relevant to the grounds for reconsideration.

Clearly highlight relevant excerpts in all your supporting documents. Do not provide entire copies of materials which are generic or already available to CPLED (i.e., your Capstone Materials, your Capstone feedback report, CPLED’s assessment criteria, etc.) without highlighting excerpts with direct relevance to your reconsideration request.
