Grounds for Reconsideration*
Set out the basis for your reconsideration request. This form must include all relevant information and documentation supporting your request. It is not sufficient to simply state your disagreement with the decision made or that you want to receive a different result. Provide a detailed statement setting out the specific facts and evidence which speak to, for example, how the decision was made in error or without procedural fairness such that a different result should have occurred. Detailed submissions regarding personal circumstances or history are not considered helpful unless such an explanation is directly relevant to the grounds for reconsideration. If additional space is required, attach extra pages.
